It's just a nickname.
I think it has something to do with the reading of the kanji in her name.

The Touhou Wiki mentions that the character for Utsuho can also mean "sky" and be pronounced "kuu".

Utsuho is a hard word to me, very misunderstanding, Okuu is easier to remember and spell.
