There is some research coming through that strongly suggests we have genes that define how easy or hard it is for us to gain muscle and get fit. In the worst cases it's almost impossible while at the other end of the scale it is fairly easy (unsurprisingly a high proportion of professional athletes seem to have genes at the good end)

They have a test they are proving the validity of and hope to make available, since on the one hand being able to tell someone that exercise will really help them could be really motivating/helpful and on the other being able to tell them they are wasting there time and should focus more on things like diet to keep weight healthy could also help.

Mikasa could simply be a "super-responder" and gain muscle like this without much effort. Although somehow I doubt the series's creators put that much thought into it ;)

Having 6 packs/muscle like that is actually not healthy despite being fit. It means your body fat percentage is really low, within the 2% range. If you want a healthy range you'd be around the 8-10%.
