I've been wondering the same thing myself. I'd almost wager that it is one person, split in two. It almost looks like there are entrails hanging out of the upper half. So many sets of what appear to be teeth too.... I'm adding the guro tag as well.

This isn't a person split in two. It's a whole person, the bottom half of the image is the rear view of what's happening from behind.
