You are right that ききょう can be some kanji other than 気胸, but it will be hard for the audiences to get the joke. Maybe I should have kept the name as Kikyou, and add a side note to it.

Well, there is no way to keep the pun when translating I think. if you just leave kikyou there ppl who don't understand Japenese will not understand that kantoku got Pneumothorax.

Maybe it should be translated as " I am Pneumothorax (which also pronounces like a girl's name in Japanese)"..

Kikyou can be the pronunciation of a couple of things in Japanese, including collapsed lung and bellflower. Kantoku is currently having collapsed lung again, and he drew this girl named Kikyou.

I was curious and looked it up. Kantoku actually draw himself as a snake like creature (post #183444). That cat is called ananeko (post #94861), not exactly sure if it is also a representation of him.
