actually, from how her rib bones are salient, id say shes actually thin, but the overall is badly drawn. so it looks like shes pregnant, but actually its only gravity on the cloth. or, shes carrying sweets inside the overall. theres no way to be sure with no tagging from the artist.

The overalls are supposed to look "billowed" or fluffed out is what the intent was probably, it's just drawn oddly.
