Yup, we are sure that this is a photoshop. Looking at the artist's name we can assume that they are Japanese, and those of us who know anything at all about Japanese porn know that they have that censorship law. Also, on this pic we can see that the genitals are not censored in any way, either by mosaic or black or white bar (the most common methods of censoring). Taking the above facts into consideration, it is therefore safe to assume that someone, somewhere has taken the original image and attempted to draw in a vagina on this person, therefore uncensoring it. We also know that Adobe Photoshop is one of the most commonly used applications for this kind of activity. Additionally, the colloquial usage of the word 'photoshop' means to use a software application (commonly Adobe Photoshop) to edit an image, which describes the activity above.

I'm well aware that the majority of of the content coming out of Japan is censored thanks to a particular public decency law. However, the fact that the artist is Japanese does not actually guarantee that an uncensored image is photoshopped. It just makes it a lot more likely. For instance, there are cases of an artist's work being released outside of Japan as uncensored with the original artist being the one who drew the part that would have been censored in Japan. So, the simple fact that something is uncensored is not enough to merit the photoshop tag even if the artist is Japanese, whereas in spite of this, there are folks who just slap the photoshop tag on uncensored images without making any attempt to verify that the image was actually photoshopped. And since the original, censored image is not on this site, and no source was given for the image, there was no obvious way to verify that the image was actually photoshopped - which is why I asked.
