I agree with your interpretation. The outlandish being, whatever the hell it is, seems to be using its slithering body to do something that, judging by the location of its tail's end and by the girl's face, is sexually pleasing to her.
Given that depictions of creepy fucks doing dirty stuff with/to humans fall under the definition of extreme_content, I think tagging the post as such is proper, even if the shock factor that is commonplace to this kind of images is somewhat downplayed by the well-placed leg and the (silly-looking) paper aesthetics.
My curiosity got the best of me and I finally clicked on this damned thing, it was blocked from view because it is tagged as guro. Talk about underwhelming.

It's a grey-area, but we could also argue why we allow monsters, mecha and landscape images here as well. As you said, it's an discussion for another day.
Monsters, mecha and landscape images can all easily fit into an anime/manga style theme, whereas this art does not fit that mold, it looks dated and out of place. I also fail to see how this qualifies as guro.
