Stark, brooding, and nightmarish. I love it. (Sometimes it nice to retreat from the comfort of the light and explore the darkest depths, y’ know).
There are innumerable things the artist gets right here. Framing. Perspective. Colour (the creature’s eyes and the back of the foreground character). Desaturation (the monochrome aspects). Depth. Shading. The tasteful application of grain filter. IMHO, the composition is perfect.
What I find disquieting is that the girl on the stairwell, standing directly in front of the creature, doesn’t appear to have any facial features. Is she a doppleganger? Is the creature somehow providing the foreground character a reflection of herself?
Without knowing the story behind the artwork, one could speculate for hours.
The longer I look at this artwork, the more disturbing it becomes.
Dismiss what I said earlier about the girl on the stairwell not having any facial features. There is one prominent feature: a single wide-staring eye. But that's not the kicker...
It seems as though the creature has the girl's head in its mouth, and is dragging her. Or, it could be starting to consume her.

The head-sucking and wandering around bit is what the source's description reads:
頭を吸って徘徊するバケモノ
But its real purpose of doing this is up to individual viewer's own guess, then (Who knows? Maybe at this moment that girl has somehow found a way to reverse-"hack" the monster, and that it's the girl herself currently controlling the monster's actions XD).

So, potentially, we could have a symbiotic relationship between monster and girl, with one trying to assert their dominance over the other. Wow, that just makes the whole scenario even more horrific.
