How do we define 'R-18'? I always thought woman's tits should be included, and obviously man's should not.

r-18 refers mostly to legal age rating, while nsfw refers to stuff that would most likely get you into trouble if seen on your screen while at work.

And... man tits should be tagged as guro, imo. :P

I'd guess it is because avatars are not subject to tag hiding/blacklists.
For example, rating:e is hidden by default but avatars circumvent this.

That'ts probably the main reason. So, until the site's system's got updated to make avatars be coherent with the thumbnails, this no-NSFW avatar rule will still be here.
